Referral Information

Medical Nutrition Therapy is generally a reimbursable service which requires a referral from your primary care physician. The referral form is printable from this website. Have your physician complete the TOP section and be sure to sign it. It can be mailed or faxed, or you can bring it with your first visit. Also be sure and bring your current insurance card.

Insurance providers may differ however, both in covered service areas and visit limitations, and you should check with your provider for your particular coverage limitations. Co-pays are accepted at the time of service.

Insurance providers currently under contract with Nutrilink or who will accept private practitioners are:

• Tufts Health Plan
• Blue Cross Blue Shield
• Harvard Pilgrim Healthcare, Inc.
• United Health Care
• Medicare Part B.

Other providers may also cover, but prior authorization may be necessary. Nutrition services are constantly being updated by the providers. Blue Cross/Blue Shield is pending service to private practitioners. Calling them is helpful to advocate for a wider selection of practitioners.

Group programs are not generally covered by third party payors, but again, you can check with your provider. The payments for workshops are due at the time of the workshop, but payments for 8 or 10 week programs can be extended over the period of the program.

Payment options and sliding scale adjustments are available for those with no insurance or limited coverage.
